Family Care Plan (DND)

Three generations of women
By identifying primary and secondary caregivers who should be contacted in the event of an emergency or military tasking, your Family Care Plan supports your family in your absence.

Your Family Care Plan  (FCP) ensures your loved ones are supported during your absence by clearly designating both primary and secondary caregivers to be contacted in case of emergencies or military taskings.

The FCP is a mandatory Department of National Defence (DND) form that ensures families of Canadian Armed Forces members are prepared for care and support in the event of unforeseen circumstances, emergency callouts, or planned deployments while contributing to individual and unit operational readiness It is a regularly updated exchange of information between your family and a Commanding Officer. The plan is designed to provide peace of mind by outlining clear arrangements, including identifying primary and secondary caregivers, for family care, allowing members to fulfil their duties knowing their loved ones will be looked after during their absence.

All Canadian Armed Forces personnel must complete the Family Care Plan (FCP) Declaration DND Form 2886. It isn’t a legally binding document, but it is kept on the military member’s file. Family Care Plans are protected under the Privacy Act.

For a copy of the "Form DND 2886, Family Care Plan (FCP) Declaration", please contact your orderly room or local Military Family Resource Centre (MFRC).
