Canada Life - Public Service Health Care Plan – Letter of Authorization
The purpose of this form, which must be completed by the PSHCP member, is to authorize a spouse or common-law partner (authorized party) to act on behalf of the member when contacting Canada Life and its sub-contractors. This authorization is only available to plan members that are unable to personally contact Canada Life and/or its sub-contractors due to their geographical location.
