Forms and Handbook for Volunteers

We ask all volunteers to review the volunteer handbook, which includes valuable information on our volunteer program, and to complete the forms required.

Once you have completed and returned all forms, we will schedule a meeting, either virtual or in person, to look at upcoming volunteer opportunities, answer questions and to officially welcome you to our volunteer team!

If this is your first time interacting with MFS-NCR, we also ask that you complete and sign these forms: Welcome Form and Privacy Notice and Consent Statement
