Where can the Annual Services Funding Application workbook be found?
For FY26-27, the Annual Services Funding Application workbooks will be distributed to each applicant via email. They are not presently accessible online.

Can the Application Declaration form be submitted without all required signatures?
Yes, Application Declaration forms can be submitted without all signatures. However, completed forms must be sent by 23 January 2026, 23:59EST via email to MFS-SQM@cfmws.com

Why are some submission fields marked as ‘if applicable’?
Applicants are reminded that not all documents related to Part C Eligibility Criteria are mandatory. Please refer to the pre-populated Funding Application Part C for what is required. Applicants must ensure that MFS has the most up-to-date version of all documents listed in Part C.

What are the indicators that a document is incorrect or has been denied?
Following the application submission window closure, any issues relating to the Applicant Submission will be communicated via email from the MFSP Stewardship & Quality Management team directly.

What actions should be taken if the application submission window has been missed?
If a Funding Application is not submitted within the application submission window, the applicant will not be eligible for FY26-27 Annual Funds.

What are the Determinants of Wellness and what do they represent?
The Determinants of Wellness are eight key areas that contribute to an individual's overall well-being. These determinants help military and Veteran families maintain resilience and withstand the challenges of military life. The eight determinants include:
- Physical: Maintaining physical health through exercise, nutrition, and medical care.
- Psychological/Emotional: Supporting emotional well-being and mental health.
- Intellectual: Engaging in lifelong learning and stimulating mental activities.
- Occupational: Achieving satisfaction and balance in work or volunteer activities.
- Social/Familial: Building strong relationships with family, friends, and community.
- Spiritual/Moral: Fostering personal values, beliefs, and purpose.
- Financial: Managing financial resources effectively and ensuring financial security.
- Environmental: Living in surroundings that promote health and well-being.

The application workbook “select all that apply” drop-down function does not appear to be working. What are the recommended troubleshooting options?
As a trouble shooting method, do ensure the workbook has been marco-enabled. You can check to ensure that macro's have been enabled by following these steps:
1. File --> Options --> Trust Centre --> Trust Centre Settings --> Macro Settings --> Enable --> OK
2. Drop-down options need to be clicked individually, and should be automatically added to your cell -
How to enable content in Microsoft Excel for the annual services application?
The Annual Services Fund Application contains a series of ‘macros’ such as drop-down options and auto-calculation features. The macros must be enabled for the workbook features to function properly.
When you open the file, the following yellow message bar may appear with a shield icon and the Enable Content button. On the Message Bar, click Enable Content. You may need to do this every time you open the workbook.

What to do if an “invalid extension” error message appears when submitting the Annual Services Funding Application?
When saving the workbook prior to uploading only, ensure that macros are disabled, as uploading an .xlsm format is not permitted. For further guidance, please refer to the video.
Important Note: Applicant's must still enable the workbook macro's to successfully complete the application. The workbook should be saved as a macro-enabled workbook until ready to submit. -
Part C - What must be submitted for the Mandatory Eligibility Criteria?
Applicants must ensure that MFS has the most up-to-date version of all documents listed in Part C. Any criteria marked as “No” must be submitted.

Part D - What information is required for the Community Profile?
The Community Profile requires information on your community’s population. See drop-down options in the Funding Application for details.

Part D - What information is required for the asset mapping and identified gaps?
The collaborative relationships section requires information on current or potential partnerships within your community. These partnerships should focus on delivering services that support military and Veteran families facing challenges related to the military and family journey challenges.

Part D - What is considered a collaborative relationship?
A collaborative relationship involves two or more organizations working together toward a common goal. In Part D of your application, list only the established or prospective partnerships that support service delivery for the three main military lifestyle challenges and three family challenges: relocation, absence, transition, mental health and well-being, finances and intimate partner relationships.

Field Description Determinants of Wellness Achieved Collaborative relationships are to be organized by primary Determinants of Wellness. In the instance where an organization achieves more than one determinant, please include it under the majority, or most appropriate. For additional information the individual determinants, see definition above. Name of Organization Provide the name of the organization offering services/resources to assist in support of the military and/or family journey challenges. Military and/or Family Journey Challenge From the drop-down list, select which military and/or family journey challenge(s) this organization is supporting. If there is more than one, please select all that apply. Type of Service Indicate the primary type of service the organization offers to military and Veteran families.
Gap AnalysisPlease indicate the identified gap(s) in service for military families this organization supports. Please indicate the proposed service delivery to be offered with the collaborated organization. Risk Analysis Identify the type of risk(s) that exists in offering this service Please identify the mitigation strategies in place to decrease the potential identified risk(s) Please identify if and what the residual risk(s) would be once mitigation strategies are in place Identify who owns this/these risk(s) (this may include the MFRC, MFS, Base/Wing, etc.) -
Why is information regarding additional sources of funds required?
A summary of anticipated external funds in support of service delivery (i.e. grants, loans, donations, base funds, etc.) is required to ensure proper allocation of public funds under the MVFSP and to better understand the financial landscape at each location should external funds no longer be available.

Where can information regarding MVFSP program eligibility be found?
Please refer to the Operations Manual, Chapter 4, for MVFSP eligible and ineligible expenses.

The overall management and administration request is showing 25% of the budget; is this okay?
Yes, for FY 26-27, although it is encouraged to keep M&A at 20-25% of your budget, MFS understands that for M&A to be reported accurately, it may exceed these amounts.
