Mess Departure Gift Form

• Complete the form and sign in the designated areas.
• You can pick up your Gift at the Mess Office in B136 during office hours 8:00-16:00.

Mess Other Gift Requisition Form 

(Marriage, NewBorn, Retirement, Sympathy, etc)  

1. Fill the form (ensure the accuracy of the info).
2. Send mess office your supporting document. * see notes below
3. The mess staff will check your eligibility for the gift and request PMC approval
4. Your E-gift will be sent to your civilian email that inputted in the form.

* notes: Send the mess office pertaining supporting documents to the with subject: Last 3 digits of Service Number-GiftType (ex. Sgt Doe with service number N12345678, subject line should be DOE678-Retirement Gift)

Gift Requisition requires necessary approval and documents.
 
 Only members in good standing are eligible for mess gift.

• Marriage- PMC Approval
• Newborn- PATA leave pass & PMC approval 
• Retirement- MPRR AND PMC APPROVAL
• Sympathy- PMC Approval
