New & renewing applicants
- Application form must be completed.
- Waivers must be completed for all applicants.
- Required photo (headshot) for your membership card. Photo(s) maybe taken on-site or sent via email. If you already have a photo on file than no photo needs to be taken at this time unless a member would like an updated photo.
- Provide proof of affiliation to determine your eligibility for this program. For example a photo of your Military/Veteran/ Employee/Government Issued ID or CFOne card (see eligibility matrix below).
- Application, waiver(s), photo(s) and proof of affiliation must be returned to PSP, once completed payment maybe made in-person or by phone. Methods of payment: in-person, debit, Visa and MasterCard; by phone: Visa & MasterCard.
- Once all documents have been reviewed and payment received, passes will be available for pick up from the Commissionaires desk at 5 CDSG Garrison St. John's at your earliest convenience.
- Application and waivers are available for pickup both in-person at Garrison Sport Stores (2nd floor) or by email.

- Monday, Wednesday and Friday - 0800-1600 hrs.
- Thursday - 0800-1900 hrs.
