The checklist is to guide you while you plan for the unexpected and routine aspects of daily life prior to a deployment or work-related absence.

Family Information Forms 

The following form is to be completed by CAF personnel (Reg/Res) and Civilian employees prior to any absence. The Edmonton Military Family Resource Centre (MFRC) will use it to gather necessary information so that we may connect with your family (locally or in other regions) in order to offer resources and support programs prior to, during and after your deployment/work-related absence. 

Please be advised the MFRC is still available to complete deployment DAG forms; however, our process has changed. Please email deployment@mfrcedmonton.com or call us at 780-973-4011 ext. 528-6300 for instructions.
